What is cellulite: 
Cellulite is the herniation of subcutaneous fat within fibrous connective tissue,  that manifests as skin dimpling and nodularity often on the pelvic region (specifically the buttock, lower limbs, and abdomen).  Cellulite occurs in most postpubescent females. Hormones play a dominant role in the formation of cellulite. Estrogens are thought to be an important hormone in the development of cellulite, and it has been proposed that an imbalance of estrogen relative to  progesterone may be associated with cellulite. 

This fact makes it a perfect fit with our hormone replacement therapy. Once the hormones are balanced, you can treat the cellulite with a lower risk of it's return. 

This shows the mechanism of action, breaks the fibrous tethers, increases  fat cell permeability, and stimulates blood flow to the basement membrane, thickening the basement membrane and preventing of the fat from herniating through the layer. 

Now for the Science.
How does this device work?
There is actual 2 distinct mechanisms behind the healing effects of acoustic wave therapy with cellulite:

1. The sound waves produce a low level of micro-trauma at the tissue level. This releases VEGF (vascular endothelial growth factor) which promotes blood vessel growth bringing more nutrients and stem cells to the area. This will thicken skin and remove dimpling.

2. The acoustic wave increases pressure momentarily, that would be equivalent to an ocean depth of approximately 2000 feet (66.7 Bar or 200 atmospheres). This releases nitrogen from the blood, which combines with eNOS (endothelial nitric oxide synthetase) and produces Nitric Oxide. Nitric Oxide is a very strong mobilizer of stem cells from the bone marrow.

This all leads to more stem cells and nutrients.
I go back to one of the basic concepts in Regenerative Medicine: “Cells not Doctors Heal Patients”. There were some surprising facts concerning the use of the shock wave device.
